2015 Where Are We With The 'Kaiser-fication' Of The System?

Several years ago I heard a smart person say that "The success of an innovation arrives when it becomes what is was created to eliminate."

In recent times we have seen the fax be replaced by email which is being replaced by text messaging which will be replaced by...

So Insurance was put in place because health care costs were accelerating too quickly and now insurance will be replaced because insurance costs are accelerating too quickly - will this solve the problem? What will replace "Kaiser-fication?"


Are we addressing the problem or the symptoms?

With education - we have already learned that "free" did not produce equal and will likely never be able to - and yet do we want it to?

Is there any value to working hard to learn how to do something and differentiating yourself because of your hard work?

Might the same value apply to health care? Might we truly focus on the "safety-net" with healthcare vs. a system that tries to handle everything?

We all have to be born - so doing things that assure healthy pregnancies and safe deliveries - benefits everyone - at least once - but does not have to be done to each person - forever.

We all have to die - so doing things at the end-of-life that allow for dignity and minimized disruption - benefits everyone - once.

In between - could we focus on healthy behaviors?

With learning - even if we provide equal opportunity to all from birth - some will still strive to be smarter - should this be prohibited? if we believe the 70-20-10 rule then only 10% of our knowledge will come to us from formal education. Will the 70-20 ever be equal? What would that look like?

2016 Would You Consider Paying Your Non-Profit Board?

The debate will continue - perhaps their needs to be an operating board of directors - much like we have today for nonprofits and a new community board - paid for by consumers - and accountable to the consumers. In my opinion both boards need to be paid - in order to attract the best talent and in order to assure accountability.

Years ago when I was more closely connected to IT training I had the chance to hear the "pitches" from training companies certified by the manufacturer of the technology and those that were certified by someone else. Which would you prefer?

My first thought was the ones certified by the manufacturer until I realized that these instructors were less likely to know the truth about competitive products -especially ones known to address deficiencies or improvements related to the core product. I changed my thinking as I valued the independence of the instructor certified by the "industry" vs. the manufacturer.

With the healthcare system - since I am ultimately paying for the board of directors - I would rather see them paid and accountable to me than to the provider.

2017 Caregivers in an Era of Value-Based Reimbursement

With healthcare - by law - and under defined situations - family members are entitled to reimbursement for care given to other members of their family. The justification seems reasonable as the economic burden to the family member providing care is a challenge (as well as the emotional burden.) These impacts could create more demands on the healthcare system.

In the Workplace - does it make sense to compensate co-workers for helping their peers learn?

We might think that the learning we gain from our "family" (co-workers, managers, customers, friends, media etc) falls in the realm of informal learning. We laugh when we talk about formalizing "informal learning." I am eager to learn more about whether this is something to be explored further. Here is a thinking model being used in healthcare related to reimbursement for family members that we may wish to consider regarding workplace learning reimbursement for "family" members.

  1. Systematically identify and support family caregivers: (who are those that we can learn from informally - how do we identify and enable them?)
  2. Use real-time data to monitor caregivers: (how can we determine if they are really helping?)
  3. Educate health care and social service professionals: (how do we prepare our L&D professionals and line managers to work with these "providers" of informal learning?)
  4. Reform payment and delivery systems: (how do we change our compensation and work flows to support the decisions made in the previous 3 items?)

How have liability, laws, payment and process changed for us as we increase the use of "telelearning?"
